Join the CSHPM/SCHPM

It is not yet possible to join the CSHPM over the internet. For the time being, we ask you sign up the old fashioned way: print out our membership form and mail it to us with your cheque to the following address:

Current Members: Please note that your dues for 2008 are due no later than mid-January.
