LETTERS OF RECOMMENDATION

You have requested that I write a letter of recommendation for you.  In order to accomplish this, I will need you to provide me with two things at least two weeks before I must put your letter in the mail.

1. CURRENT COPY OF YOUR RESUME.     (You can attach to an email.)

2. AN EMAIL CONTAINING THE FOLLOWING INFORMATION:

        A.  For what are you applying? Scholarship, Graduate School, Employment, Etc.

        B. List the program(s) to which you are applying.
                    Please include the following:
                        1. Name of the program.
                        2. The address to which you want the recommendation sent.
                        3. The date by which I need to have the recommendation mailed. Allow at least a week to
                            account for the time it takes the mail to be delivered.

        C. List the classes you have taken from me, the semesters in which you took them, and your final grade.

        D. Is there anything that is not mentioned in your resume of which you feel I should be made aware?
 

Please send me email reminders as the deadline approaches.  I will send you an email when I have mailed the letter.  Feel free to drop by my office to discuss your letter at any time.  Best of luck with your application.
